Job Title: Spanish Speaking Case Manager
Location: Bronx (Mott Haven), NY — On-Site & Field-Based
Client: Center for Urban Community Services (CUCS)
Job Type: Full-Time, Direct Hire
Schedule: Monday – Friday, 9:00 AM – 5:00 PM
Compensation: $44,240.56 – $59,930.96 / year
Position Summary
Randstad is seeking a Spanish Speaking Case Manager to join CUCS supporting the Betances Residence (445 E. 142nd St, Bronx). Serving 152 low-income and formerly homeless older adults (55+)—many living with mental illness, HIV/AIDS, or substance use histories—this role delivers person-centered care to a dedicated caseload. Working on-site and in the community, the Case Manager provides direct outreach, crisis intervention, benefit coordination, and skill-building to help clients maintain housing stability and self-sufficiency.
Key Responsibilities
Case Management & Escorts: Develop individualized service plans; conduct home visits, community appointment escorts (medical, legal, benefits), and medication self-administration support.
Benefits & Financial Coordination: Help clients secure and renew public benefits (SSI/SSDI, Medicaid, SNAP, HASA), manage payee funds, and track financial goals.
Housing Stability & ADL Support: Assist with move-in orientation, lease recertifications, inspection prep, landlord relations, and daily living skills (hygiene, cleaning, hoarding mitigation).
Crisis Response & Operations: Provide crisis intervention, facilitate support groups, coordinate emergency services, and complete EHR progress notes to meet monthly contact targets.
Qualifications & Requirements
Language: Proficiency in Spanish and English required (verbal and written).
Education: High School Diploma or GED required (Bachelor’s in Social Work, Psychology, or related field preferred).
Experience: No formal experience required (prior exposure to supportive housing or vulnerable populations preferred).
Core Competencies: De-escalation skills, strengths-based assessment abilities, person-centered care, and comfort with community visits.
Benefits Summary

Responsibilities
Client Engagement & Service Delivery: Develop service plans (ILPs), provide regular outreach, support medication self-administration, and escort clients to medical, legal, and benefit appointments.
Benefits & Housing Retention: Help clients secure public benefits (SSI/SSDI, Medicaid, SNAP), manage rep payee funds, handle recertifications/inspections, and assist with daily living skills (ADLs, cleaning, pest remediation).
Crisis Response & Operations: Deliver trauma-informed de-escalation, coordinate emergency care, run support/recreational groups, and maintain timely EHR progress notes for client contact targets.
